com/join-our-team/ Assists with planning and implementing a comprehensive social services program which provides for counseling and other support services for residents.
Acts as a resident advocate by providing counseling for residents and responsible parties, processing Social Service referrals, assisting residents and responsible parties to resolve complaints.
Interviewing all residents and/or responsible parties to complete Social Service assessment and related sections of MDS and Plan of Care forms; completion of Advance Directives.
Assist in completing admission paperwork and discharge planning for residents as needed.
Performs initial assessment of each resident's social needs then formulates the plan for providing care and records the plan in the resident's medical record.
Periodically re-evaluates in conjunction with the resident's total plan of care.
Attends/participates in POC (Plan of Care) conferences; writes quarterly reviews; updates Social Service section of Plan of Care, other applicable areas and MDS as required per significant change in conditions.
Identifies medical-related social needs of residents, arranges, schedules and coordinates resident podiatry, psychology, vision, psychiatry, audiologist, hospice and dental appointments at the facility and services provided outside of the facility per resident needs/requests.
Ensures residents have necessary personal items; contacts responsible parties to arrange for delivery of needed or requested items, etc.

Attends/participates in RIFT, Family Meetings, and Grievance Meetings with Compliance coordinator, assists psychiatrist and psychologist as needed.
Involved in assessing resident room assignments; notifies responsible parties of room changes after receiving permission from resident.
Provides backup for Admission/Discharge Coordinator functions.
Acts as liaison between residents, families, outside agencies and the Director of Nursing to ensure that the resident's rights are maintained.
Leads and takes minutes for family council and grievance committee meetings; conducts interviews for grievances, complaints and forwards necessary documentation to appropriate department; Organizes and leads Gables specific Ethic Committee; participates as ad-hoc in ethics committee meetings for Memorial Hospital of Union County; Assist in giving tours and information to prospective clients as needed.
Provides back up for and works with Admission/Discharge Coordinator on residents discharge from facility or other ancillary services starting at the time of admission.
Works directly with residents and their families experiencing personal and environmental concerns related to the resident's physical or emotional condition; One on One counseling for resident related issues, i.
e.
, dealing with a terminal illness; complicated family decision making etc.
Works with team in marketing services.
Requirements Completion of Bachelor's degree in social work, sociology, psychology, or related field and/or equivalent combination of education and experience.
Encouraged to obtain and maintain a license as a Social Worker in the state of Ohio.
Must possess a valid Ohio driver's license.
